Important Notice:

The Ohio Department of Aging (AGE) is working to transition to the Systems for Aging (S-AGE) provider portal.

In order to gain access to S-AGE and the Dynamic Data Exchange (DDE), you must supply your agency and user information by completing this survey. This information includes use of an OHID number to access the system.

If you have completed the survey and set up an OHID, then no further action is required. If you need to create an OHID, please refer to the instructions starting on page 3 of this Job Aid.

Important: DDE Transition to S-AGE

As part of the transition to the new Systems for Aging (S-AGE) Provider Portal, providers should complete and submit all required activity in DDE before 4:00 PM on Thursday, October 1, 2026. DDE will no longer be available for normal business processing during the cutover period.

The S-AGE Provider Portal will go live Monday, October 5, 2026. Beginning October 5, please use S-AGE for applicable provider activities, including invoice submission.

Please refer to the S-AGE onboarding communications you received for additional transition details, training information, access instructions, and the direct link to S-AGE.

Access Requirements

OHID Account: A valid numeric OHID is required to access S-AGE. Use the same OHID previously provided to AGE for your S-AGE account. Need assistance? Visit the OHID Help Center.
Required eLearning: Provider users must complete their assigned eLearning before S-AGE access is activated.
