The Ohio Department of Aging (AGE) is working to transition to the Systems for Aging (S-AGE) provider portal.

In order to gain access to S-AGE and the Dynamic Data Exchange (DDE), you must supply your agency and user information by completing this survey. This information includes use of an OHID number to access the system.

If you have completed the survey and set up an OHID, then no further action is required. If you need to create an OHID, please refer to the instructions starting on page 3 of this Job Aid.

Important Notice:

The Ohio Department of Aging (AGE) is transitioning to the Systems for Aging (S-AGE) portal. To access S-AGE, you must have a numeric OHID account and be provisioned for system access.

After logging in, you will be required to complete eLearning modules. Completing these modules will help ensure a seamless transition to the new system.

Please note: Your eLearning access is tied to this specific OHID. If you use a different OHID, your assigned modules will not appear.
